College academic - Tutorial change request form

This form is to be filled if you have to make a tutorial change in a UOW College academic course and are unable to do so via SOLS.

Please do not use this form if you are a UOW, Vocational, or English Language student.

Instructions:
1) Please complete all required fields in the form. Fields marked with an asterisk (*) are mandatory.Please provide up to two tutorial preferences you would like to move to for this subject.
2) If we are unable to place you into your first preference, we might be able to place you in your second preference.

Please note:
1) Changes cannot be made after Week 2 of session.
2) If you wish to move tutorial for a subject that has multiple tutorials throughout the week then ALL the classes for that tutorial throughout the week will be change to your preferred tutorial number.
3) Completing the form doesn’t guarantee that your request will be approved due restrictions on class sizes and other factors.
4) You will be notified of the outcome via SOLS/email.
