Course Withdrawal Request Form

Purpose:

This form should be filled if you (a) are not returning to study after your Leave of Absence period, (b) International students discontinue studies to return to home country, (c) transferring to another institution and UOW has approved a release from your UOW degree.

Instructions:
1. If you are discontinuing studies with UOWCA to return to home country (International Student Guidelines)
- Complete this Course Withdrawal Form while giving a brief reasoning to explain why you cannot continue studies.
- Prepare support documentation: a copy of your boarding pass OR a copy of your flight ticket plus the receipt showing that your ticket has been paid in full.

2. If you are discontinuing studies with UOWCA and transferring to another institution (International Student Guidelines)
- Step 1: You must maintain enrolment during the application process so that you are not breaching your student visa conditions.
- Step 2: Apply for a release from UOW if you have UOW degree offer.
- Step 3: Complete this Course Withdrawal form if you are abiding by Step 1 and 2.
- Step 4: Prepare and Attach Supporting Document: a copy of the official offer letter from the new university, one detailed letter of your compassionate or compelling circumstances, if you are a government-sponsored student, a copy of sponsor's letter supporting your leaving UOWCA for another institution.

3. If you are discontinuing studies with UOWCA and transferring to another institution (Domestic Student Guidelines)
- Step 1: Prepare and Attach Supporting Document: a copy of the official offer letter from the new university, one detailed letter of your compassionate or compelling circumstances.
- Step 2: Complete this course Withdrawal form
