Who should use this form?

This form must be completed by Under 18 students who seek a temporary absence from UOW College homestay arrangement and for travel purposes.
When should this form be submitted?

This form must be completed and submitted no later then 1 (one) week before student's intended date of departure from homestay.
What supporting documents is required?

Student must provide the supporting documents listed in Part 4 and Part 5.
