Who should use this form?
This form is for students who may be eligible to access Smart & Skilled funding (in addition to other funding such as VET Student Loans.)
Smart and Skilled is a NSW Government funding incentive that provides eligible students who wish to gain new skills to find a job or advance their careers with an entitlement to subsidised training.
You may be eligible for Smart and Skilled funding if you are:
• Aged 15 years or older;
• No longer at school;
• Live or work in NSW; and
• An Australian citizen, permanent resident, humanitarian visa holder or New Zealand citizen.

Examples of suitable evidence
Proof of Receiving Welfare
• A letter from Centrelink confirming receipt of the benefit. The letter should clearly show the Centrelink Reference Number (CRN) and benefit or allowance category, OR
• A current concession card that shows the CRN and clearly shows the benefit or allowance category, OR
• A current Centrelink income statement that clearly shows the CRN and the benefit allowance category, OR
• Any other evidence that clearly shows the CRN and the benefit allowance category, OR
• Documentary evidence from the Department of Veterans Affairs stating their pension/benefit status, OR
• For people applying for Austudy or Youth Allowance, an approval letter from Centrelink that shows the CRN and indicates that the commencement date of their benefit is within two weeks of their enrolment or two weeks with the date of the first class attendance or participation in training.

Proof of Disability Status
• A letter from a medical practitioner, school counsellor or special education coordinator, Disability Service Provider or Job Capacity Assessor, or specialist allied health professional (Including rehabilitation counsellor, psychologist, speech pathologist or occupational therapist) stating that, at time of enrolment, you are a person with a disability and a current client of this specialist professional.
• Department of Veterans’ Affairs Gold Repatriation Health Card indicating that you are in receipt of an Extreme Disablement Adjustment (EDA) or Total and Permanent Incapacity (T&PI) payment
• A current Department or Human Services (Centrelink) Income statement.
• Evidence that the benefit provider currently accepts you as the dependent child, spouse or partner of a beneficiary receiving a benefit or allowance.

UOW College Australia (UOWCA) is committed to protecting and maintaining you privacy and the security of your personal information. The information you provide in this form will be managed in accordance with the UOW Global Enterprises Privacy Policy which can be viewed on our Policy and Procedures webpage.
Why we collect your personal information
UOWCA collects your personal information so we can process and manage your enrolment in a Vocational Education and Training (VET) course and to satisfy our data reporting requirements as a registered training organisation (RTO) under the Standards for Registered Training Organisations (RTOs) 2015. If you do not provide the personal information requested, UOWCA may not be able to proceed with your enrolment.
In accordance with the Data Provision Requirements under the National Vocational Education and Training Regulator Act 2011 (NVETR Act) and the Department of Education and Workplace Relations (DEWR) National VET Data Policy, UOW College Australia (UOWCA) is required to meet data collection and reporting requirements.
Why we may disclose your personal information
We are required by law (under the NVETR Act) to disclose information we collect about you as a VET student (including the personal information contained on this enrolment form) to the National Centre for Vocational Education Research Ltd (NCVER). NCVER is the national professional body responsible for collecting, managing, analysing and communicating research and statistics on the Australian VET sector.
We are also authorised by law (under the NVETR Act) to disclose your personal information to the relevant state or territory training authority.
How is your personal information used?
We use your personal information to enable us to deliver VET courses to you, manage your enrolment and to comply with our obligations as an RTO.
Your information may be used by NCVER or a government department for purposes that include: producing statistics and research relating to education; to help policy makers and researchers gain a better understanding of the VET sector; understand how the VET market operates; support critical policy and other decision making; and shed light on evolving priorities in the VET sector.
Your personal information may also be disclosed to a third party contractor or other authorised agency, for research or to collect further information via a linkage with other data collections and to deliver student surveys.
How do NCVER and other bodies handle your personal information?
NCVER will collect, hold, use and disclose your personal information in accordance with the law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act. For more information on how your privacy is managed by NCVER go to: https://www.ncver.edu.au/privacy
NCVER is authorised to disclose information to the Australian Government Department of Employment and Workplace Relations (DEWR), Commonwealth authorities, State and Territory authorities (other than registered training organisations) that deal with matters relating to VET and VET regulators for the purposes of those bodies.
The Department of Education is authorised by law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act, to collect, use and disclose your personal information to fulfil specified functions and activities. Consent
In providing your personal information as requested and proceeding with your enrolment or re-enrolment, you provide UOW College Australia with your consent to collect, store, use and disclose your personal information as outlined, and in accordance with the UOWGE Privacy Policy and the below primary purposes, and/or any secondary purposes reasonably related to the below primary purposes which include:
• The administration of your application, enrolment, course progression, graduation, conferral of awards and the provision of student services to you by UOW College Australia and the University of Wollongong;
• Reporting and verifying or exchanging your information, pursuant to UOW College Australia and UOWs obligations under Australian or state law, including:
o Australian Commonwealth and State Government departments and agencies, with responsibility for immigration, human services and/or education;
o The Australian Skills Quality Authority (ASQA);
o Job Services Australia Providers;
o Host Organisations delivering Student Work Placements (for the purpose of gaining professional experience and knowledge as a formal requirement of your UOW College Australia course).
