The information you provide on this form is being collected by UOW to assess applicants for the UOW Elite Athlete Program. Your personal information is held securely on University systems that are subject to technical and organisational measures to protect against unauthorised or unlawful access and against accidental loss, destruction or damage. Your personal information may be shared with relevant UOW units/staff to provide you with the above services. We will not share your personal information with any other third parties unless we have your consent or there is a legal requirement to do so. UOW has an Agreement with the Australian Institute of Sport’s (AIS) Elite Athlete Network. Under the terms of that Agreement, if a student is categorised as being an Athlete, for the purposes of the EAEN Agreement, a person deemed by the AIS to be an elite athlete according to the AIS Sport Categorisation Table, UOW may share the following personal information with the AIS to enable them to support these athletes and monitor their progress:
• Full name
• University Campus
• Course
• Sport
• Contact details of relevant contact person within the sport (Typically the AW&E Manager)
• Scholarship Status
• Commencement Date
• Anticipated Finish Date
• Delivery mode (on campus/online)
• Study Load (FT, PT)
• Home Institution (NIN, NSO)
