Student Wellbeing and Support Privacy Disclosure and Consent Form

Privacy Notice:

UOW College Australia is committed to protecting and maintaining the privacy of your personal information. The information provided in this form is collected for the primary purpose of providing UOW College Australia with your consent to disclose information to the identified ‘Authorised Support Person' and will be managed in accordance with the organisation’s Privacy Policy which can be viewed at www.uowcollege.edu.au/support-resources/policies-procedures/.

Information for Students:

UOW College is committed to ensuring that students are provided effective support during the period of their enrolment. On occasions, the UOW College Australia General Manager or Deputy General Manager may consider that disclosure of information to an ‘Authorised Support Person’ is important to provide students with additional support or advice. By electing to complete this form, students can nominate an ‘Authorised Support Person’ (i.e. a parent/guardian or other close personal contact) and authorise UOW College Australia to contact the nominated ‘Authorised Support Person’ in certain circumstances.

Disclosure of personal information to the ‘Authorised Support Person’ nominated on this form may occur in the following circumstances (being circumstances where personal support or advice from the ‘Authorised Support Person’ may be suitable or beneficial to the student):
• where a student has not met the requirements for satisfactory course progress (refer to the UOW College Student Progression and Support Policy); or,
• where a student has not met the requirements for satisfactory attendance (refer to the UOW College Attendance Policy); or,
• where a student has not maintained the Standards for Student Conduct (as outlined in the UOW College Academic Integrity and Student Conduct Policy); or,
• in exceptional circumstances where the UOW College Australia General Manager or Deputy General Manager believe that the disclosure of a student’s personal information to the ‘Authorised Support Person’ is important for the student’s health, safety, or wellbeing; or,
• where issues arise concerning fee options, payments or other financial matters which may have a significant impact on a student’s enrolment at UOW College Australia.

The ‘Authorised Support Person’ nominated on this form cannot represent the student and is not permitted to access any other information pertaining to the student’s enrolment. The ‘Authorised Support Person’ nominated on this form is also not permitted to make any changes to the student’s information or enrolment at UOW College.

By signing this form, I authorise UOW College Australia to contact the ‘Authorised Support Person’ nominated on this form and disclose my personal information in the circumstances set out above where the College considers that the disclosure to the above nominated ‘Authorised Support Person’ is important for the provision of additional independent and/or personal support and/or advice to me.

I understand that this consent is valid for the duration of my enrolment as a student with UOW College Australia and that I can withdraw my consent at any time.
